This happens frequently. The connection was invalidated. You need to reconnect by creating a new connection and adding it to your Flow or fixing the current one.
When you try to do the new connection you'll be asked for the MFA and this will solve the issue.

MFA was removed but the issue has come up again. Can you explain how to fix an existing connection where I'll be asked to MFA?
Our users are downloading the Microsoft authenticator app and using that to sign into their office.com and email accounts. I'm not sure how if this will break the flow each time it tries to run?
Is there any documentation on how to handle this issue?

There are two ways to refresh the connection. If you want it only for your Flow, then you can add a new connection, and you'll be prompted for the MFA.
Otherwise, the best way is to go to "Data" > "Connections," and then probably you'll see something like this:
Fix the connection, and you'll be prompted for the MFA.

Thanks for your help on this.
My problem is that the connections are breaking as such.
I have other users trigger my flow from a powerapp and those users that have MFA enabled are experiencing flow failures. When I log into my flow I don't see any broken connections.
I never shared the flow with the users for security reasons. Do those users need to go into Power Automate and fix their connections?

Unfortunately yes. They will have their own connections, and if something changes in the tenant, it will affect everyone.
Even if the connection is not invalid, I'm almost sure that your issues come from that.
As a test, create a new connection and change it in one Flow. If it solves your issue, then we have a working connection. If not, then we need to dig further.
