Sharepoint Create File Dynamic Field Not Working

I have this flow: 

And this error: 

The issue is the dynamic field "04 - Subcontractor Name" in the Sharepoint File Name field... it isn't going through to the actual file name and as a result the Flow is trying to name all submissions on a given date identically because the unique identifier (the Subcontractor Name field) is failing. 

I've swapped that field out with the Form ID field and the flow then works. But I really need the Subcontractor Name as part of the file name otherwise I have to open each file and rename it and it kind of defeats the purpose of the flow in the first place. 

I have an identical form and flow in place for another region and that one works perfectly fine with the Subcontractor Name field in the File Name. 

Any advice on this would be greatly appreciated!

Can I ask which Connector trigger you are using?

Thanks for your question. The trigger for the flow is when a form is submitted on jotform. Is that what you mean? Thanks.
