Update an existing line in our SharePoint List using Forms

Hello,

I am trying to make it when a location fills out a form it will update an existing line in our SharePoint instead of creating a new line. We have tried many options with no success, can anyone help we feel this should be very simple.

The form is very easy, they pick their region, then it asks them for their store then it asks if they completed the select task and they click on yes.

The list was made with three columns in mind to these answers and all we want is the last one updated with a "Yes" to the existing column.

When the MS Form is submitted, which item in SharePoint do you want to update based on the Form's answer?
