How can we save modified by and when each time a field changes for each field onn the app?

Lets say multiple people provide input to fields on one form.

1st. Is the car clean? ________ Modified by Jim and when 4/11/2019 8am

2nd. Is the car filled up with gas?_____Modified by John and when 4/11/2019 9am

3rd. Was the car damaged?_____Modified by Judy and when 4/12/2019 7am

Jim says 1st field in complete

John says 2nd field in complete

Judy say 3rd field is complete

Form modified 3 times by 3 different users... Want to keep last modified per field.
