Building a Survey where the responses have a yes or no selection and the answers are stored in dataverse table in same row with question

Hello, 

Im trying to develop a power app has a list of questions with a "Yes" or "No" selection. The Questions are hard coded and I want users to interact and answer Yes or No. I want this to interact with a Dataverse table and their selections to be placed into the table after filling it out. Can someone help out with how you would approach this question!
