I've created the columns for the Location table. When I go to the form, the LocationImage field is not available as an option to add to the form. The columns page and the forms page are attached.

You can check the environment on the upper right side of the screen.

I am also having the same problem. I have been working in my dedicated environment.
I have the same problem. It is available in views but not in forms.
I experienced similar issue, I created a new location image column in the form environment. This can be done by clicking on the +New table column at the upper part of the left-hand side panel. I hope this helps.

This seems to be a bit of a bug as the column should show up as expected. However, when testing I ran into the same issue.
I found a way to make this work by deleting the column from the solution and then, in the form editor, select New table column and then add the column using the same steps as when originally creating the column.
As you can see below, this made the column available for me to add to the form:
same issue for me and when i try to delete the column from the table it says dependencies exist
