Customize a new form in Sharepoint list with specific columns are grouped by Group name

Dear all, 

I'm a newbie with Sharepoint list & Power App .

I created a Sharepoint list and I'm targeting to create a display when click on "+New item" in Sharepoint list with specific columns grouped under each Group name (Details, Approver information, Other) as image below. This is a target display. If you know how to create this display, kindly give me some instructions. Many thanks for your support

Do you want to create multi forms to submit the same record?

Actually, you just need to create a special tab form.

Here is a simple demo for your reference.

1). First of all, please go to the list settings and select the form settings, choose the "Use a custom form created in PowerApps".

2). Create Labels to set the Text property as the group name you want to display within, copy the sharepoint form control and add your needed fields in each group.

3). Set the following properties of the SharePointIntegration:

ResetForm(SharePointForm1);ResetForm(SharePointForm1_1)// OnCancel

EditForm(SharePointForm1)&&EditForm(SharePointForm1_1)//OnEdit

NewForm(SharePointForm1)&&NewForm(SharePointForm1_1);Set(varSelected,Blank())//OnNew

If(IsBlank(varSelected),Patch(ListName,Defaults(ListName),SharePointForm1.Updates,SharePointForm1_1.Updates),Patch(BU,SharePointIntegration.Selected,SharePointForm1.Updates,SharePointForm1_1.Updates))//OnSave

ViewForm(SharePointForm1)&&ViewForm(SharePointForm1_1)//OnView

4). Set the Item property of each SharePoint form control:

Thanks for your instruction, I followed these steps in Power app => save & publish however I don't know why it is not applied when I create a new form in List

It's based on your app configuration.

Don't know what you have done in your scenario, I could not imagine this. At least, you could tell me what's the story for now.

Thanks for your quickly reply, 

I would like to summarize the steps which I performed below:

- Firstly I went to my List => New Form => Customize with Power Apps

- Secondly, I update a form in Power Apps with design below and Click on Save => Publish

- And then, I go back to my List on Sharepoint, select List setting => Form Settings and choose "Use a custom form created in PowerApps"

- However, when I create a new form in List it still shows the default form not customized form which I created in Power App 

I tried to publish many times but it not work

1). Make sure you have save the settings, click the OK button.

AND please set this at the very beginning of your test.

Hi @v-qiaqi-msft,

I tried again to set Form Setting => Ok => Editing form in Power App and Publish it however, it still not works. I just choose the App checker on Power App and see errors in Accessibility (Missing Accessible Label), I don't know whether these errors led to my form can not be applied to List

That's not the key.

I think this could be a cache issue. Try to create a new list and use the custom form directly.

Thank you so much for your support & instruction, I do it again with 1st step in Form Settings and then create a Custom form and it has been already applied for a New form/Edit form in List.
